Any certificate program must be compatible and consistent with university enrollment management plans.
For example, when originally proposed it was anticipated that the Dosimetry Certificate program might attract students who were established professionals but who had received their radiology technology training through hospital based programs. Such individuals might not posses academic degrees such as a bachelors or associate and consequently not formally match a university admissions category. The policy is designed to ensure that certificate programs have admissions requirements consistent with those for degree seeking students and yet provide flexibility where appropriate. In such cases, certificate proposals should describe the anticipated student population and why this should be considered consistent with university admissions requirements.
Certificate programs that are totally credit-bearing or are the combination of credit bearing and non-credit bearing courses will, after approval by the proposing department and any required college level review, be reviewed by the Academic Planning Committee, the Undergraduate Curriculum Committee or the Graduate Curriculum committee and the Faculty Senate.
Once approved certificate programs will be reviewed as a part of the academic program review of the home department. This review will report on the extent to which the program has met the standards.
A non-credit-bearing certificate program will originate with an academic department. The certificate program must be endorsed by all affected departments, be approved at the college level and by the Provost.

LX 137 - Summary of Proposed Changes
Summary form to be used when departments are proposing numerous changes.
LX 138 FormsThere are two LX 138 forms used to record curriculum changes - one for courses (LX 138C) and one for programs (LX 138P).
LX 138P - Program Curriculum Change (yellow/goldenrod)
Form used to make changes to majors, minors, concentrations, emphases and college core. Changes include credits, required courses, electives, and other program requirements. It is also used for submitting new program curricula to the curriculum committee. Other committees that review new programs (e.g. - Academic Planning Committee) may require different materials. You may attach details instead of listing on form.
LX 138C - Course Curriculum Change (yellow/goldenrod)Form used to make any change to a course including description, rerequisite(s), credits, title, grading pattern, instructional pattern, course number or delivery options. This form is also used to delete a course and to create a new course. If proposing a new course, page 3 of the form must be completed.
This form includes several check-off items to guide departments when considering various aspects of a course proposal/change.
LX 139 - Course Outline (green)
Form used to provide course objectives, outline and requirements. It is never used by itself; it accompanies the LX 138C. It must be submitted for all new courses. Some changes submitted on an LX 138C might not require an LX 139; although it is logical that a LX 139 would be completed when changing the description, instructional pattern or number of credits.
LX 140 - General Education Course Change (green)
Form used to propose a course to be considered part of the General Education program or to make changes to an existing General Education course. It is a companion to the LX 138C. This form does not need to be presented to the curriculum committee(s); it is for General Education Committee use.
LX 140D - General Education Course Deletion (green)
Form used to delete a course form the General Education program. Submit LX 138C to permanently delete a course from the course catalog.
